Team Registration Information
To participate in Canyon Athletic programs (school year & summer), student must have a valid physical, complete a athletic clearance account and complete the Canyon summer program registration form. Information on how to complete this process is as follows:
Physical Screen
Receive a pre-participation physical examination. Physical examinations are valid one calendar year. A copy of the OUSD Physical Exam form is as follows:
Athletic Clearance Registration
All athlete must register for Athletic Clearance and complete a new clearance for each sport they play or tryout for. Once completed the Athletic Clearance Confirmation Page should be emailed or returned to Mrs. Medina (dmedina@orangeusd.org). Tips to help you get started are below:
- Go to www.AthleticClearance.com
- Incoming students register for an athletic clearance account
- Returning students log in to your current athletic clearance account
- Create a “New Clearance” for each sport you play
- Complete Athletic Clearance steps 1-4:
- Step 1: Student Info (upload physical)
- Step 2: Medical History
- Step 3: Parent Guardian/Information
- Step 4: Signatures
- Step 5: Donations are collected by each program.
- Print a copy of your Athletic Clearance Registration Confirmation page and turn in to Canyon’s athletic trainer, Mrs. Medina.
Canyon Summer Program Registration
Create a www.athleticclearance.com account for the 2022-2023 school year. Starting March 1st you may register for each sports camp your athlete will be participating in during the summer and school year.
Print and upload the signed confirmation page along with a valid physical to your account. Register by May 1st. Please allow up to 2 days before summer camp begins for staff to review and clear your student’s account.
