Student Accident Report: This form is to be completed by athletic trainers, coaches, or other supervising school personnel in the event a accident occurs during Canyon High School related activities. This form should be turned into the Canyon High School health office.
Hospitalization/Injury/Extended Medical Absence Letter: Often times after an injury requiring hospitalization or a prolonged absence, students may feel overwhelmed trying to catch up and/or may be unable to keep up with the current pace of their classes.
